      <title>New Citizenship Documentation Option for Medicaid and CHIP is Up and Running</title>

      <description>Policy brief on the Children’s Health Insurance Reauthorization Act (CHIPRA) and the reduction of the detrimental effects of a Medicaid citizenship documentation requirement enacted in 2006 that has caused many eligible citizens to lose or be denied health coverage. The CHIPRA provision gives states the option of meeting this requirement by conducting a data match with the Social Security Administration’s (SSA) database to verify an applicant’s U.S. citizenship. States that have taken up this option, which became available on January 1, report that it eases the paperwork burden on families applying for coverage, simplifies enrollment procedures, and produces significant administrative savings.  The new health reform law requires states to use this data-matching system to verify citizenship for people who seek coverage in the new health insurance “exchanges” that will be established. The early success of this system suggests that it will work well when individuals apply for subsidies and health coverage through the exchanges. This system also can help states coordinate the eligibility determinations they will conduct for Medicaid, CHIP and the new health reform subsidies.</description>

Over the course of two decades, California has seen an increase in the percentage of people who are uninsured as the proportion with employer-sponsored coverage has contracted. Between 1987 and 2008, the segment of the non-elderly population covered by insurance provided through an employer shrank by 9 percent. Although increases in Medicaid coverage partially offset the decline, more than 20 percent of Californians remain uninsured. Though the problem is national, California has both a lower percentage of individuals with employer-sponsored coverage and a higher proportion of uninsured. California also has the largest number of people without insurance — 6.6 million — of any U.S. state.</description>

      <description>This memorandum details the Governor&apos;s recommendations to address the State&apos;s estimated revenue shortfall of $11.2 billion in FY 2008-2009. Specific health care cuts include reductions and changes in the: Federal Safety Net Care Pool Payments, Medi-Cal 1931(b) Program, Medi-Cal Monthly Reporting Requirement for Undocumented Immigrants, Medi-Cal Eligibility for Legal Immigrants and Medi-Cal Share of Cost. Additionally, payment amounts for SSI/SSP, In-Home Supportive Services (IHSS) and CalWorks would be reduced and the Cash Assistance Program for Immigrants (CAPI) would be eliminated. Many of these cuts were introduced in the FY 2008-2009 May Revision but were rejected by the legislature.</description>

It goes on to offer impact results from a survey of 300 health centers nationwide, with 90% reporting enrollment difficulties; many also reporting longer application processes, increased staff time to assist with applications, and delays in their ability to arrange for specialty care. It estimates that the impact of citizenship verification will eliminate Medicaid coverage for 2.2 - 6.7% of all Medicaid enrolled patients (approximately 212,400 children and 107,100 adults).</description>
