County DHS to the Board of Supervisors requesting Board approval to amend an agreement with Health Management Associates (HMA) for consultant services to assist DHS with its ambulatory care restructuring and other initiatives to implement the California Medicaid 1115 Waiver such as ambulatory care/managed care restructuring; coordinating care among DHS, Department of Mental Health (DMH) and Department of Public Health (DPH); DHS interaction with correctional health; clinic productivity cost review; Medical Home 90/10 Initiative and development of lower level of care relationships. The proposed amendment will extend the agreement for six months through June 30, 2012.
