Chronic Conditions

Chronic diseases present numerous difficulties and challenges to individuals and health care systems. Death rates from these conditions often do not reveal their influence on disease burden. People are living longer with chronic conditions such as cardiovascular disease and diabetes, which results in an increased need for medical care and disease management programs. Significant health disparities exist within L.A. County. SPAs that fare far worse on several key chronic disease indicators include SPA 4 (Metro), SPA 6 (South L.A.) and SPA 7 (East L.A.).
